In order to make your first experience with the office of William A. Atlas, MD a pleasant one, please download and complete the following forms (if applicable) by the time of your first visit. This will assist our staff in making sure we have all the necessary information in time for your scheduled appointment.
Please complete the following forms and present them to our staff when you arrive or you may fax them to our office at 210-404-9223. In addition, please bring a list of your medications, a picture ID and your insurance card(s).
